ABSTRACT

@#Objective     To investigate the clinical characteristics and risk factors for perioperative lung surgery patients with SARS‐CoV‐2 Omicron variant infection. Methods     The clinical data of patients who underwent lung surgery at the Department of Thoracic Surgery, Renmin Hospital of Wuhan University from December 1, 2022 to January 9, 2023 were retrospectively analyzed. The patients were divided into an infection group and a non-infection group according to whether they were infected with SARS-CoV-2. And the clinical data of two groups were collected and compared. Multiple linear regression analysis was used to explore the risk factors affecting the time of hospitalization. Results     A total of 70 patients were enrolled in this study, including 36 (51.4%) males and 34 (48.6%) females at a median age of 61.0 (49.0, 66.8) years. There were 28 patients in the infection group and 42 patients in the non-infection group. The proportion of preoperative abnormal coagulation function and the risk of postoperative pulmonary infection in perioperative patients infected with SARS-CoV-2 were higher than those in the non-infection group (P<0.05). Subgroup analysis found that patients with preoperative SARS-CoV-2 infection were more likely to have pulmonary infection after surgery, but did not prolong the time of hospitalization or increase the risk of severe disease rate. The patients with postoperative SARS-CoV-2 infection had worse clinical prognosis, including longer time of hospitalization (P=0.004), higher ICU admission rate (P=0.000), higher lung infection rate (P=0.003) and respiratory failure rate (P=0.000). Multiple linear regression analysis showed that gender and extent of surgery were independent risk factors for prolonged hospitalization time. Conclusion     Preoperative infection with SARS-CoV-2 Omicron variant will increase the risk of pulmonary infection, but it will not affect the clinical prognosis. However, postoperative infection with SARS-CoV-2 Omicron variant will still prolong the time of hospitalization, increase the ICU rate, and the risk of pulmonary complications.

ABSTRACT

Objective:To explore the application value of metagenomics sequencing in the etiological diagnosis of bacterial meningitis(BM) in children and improve the diagnosis and treatment effect of BM in children.Methods:All BM cases were collected from Jiangxi Provincial Children′s Hospital from February 1, 2018 to January 31, 2020.Meanwhile, such biological samples as blood and cerebrospinal fluid were collected for traditional etiological testing and metagenomics sequencing.The results of traditional etiological testing were regarded as the gold standard, and the specificity and sensitivity of metagenomics sequencing in the diagnosis of BM in children were confirmed.Results:A total of 45 cases were collected in this study, including 31 males and 14 females, with the age between (74.74 ± 58.67) months.Twenty-six cases were identified by metagenomics sequencing, with the positive rate being 57.78%.Among them, there were 8 cases of Streptococcus pneumoniae, 2 cases of Escherichia coli, 2 cases of Neisseria meningitidis, 2 cases of Staphylococcus, 2 cases of Salmonella, 2 cases of Mycobacterium multiplex, 1 case of Streptococcus intermedius, 1 case of Streptococcus pyogenes, 1 case of Streptococcus paris, 1 case of Streptococcus salivarius, 1 case of Haemophilus influenzae, 1 case of Pseudomonas aeruginosa, 1 case of Acinetobacter baumannii, and 1 case of Aspergillus.The traditional etiological positive rate was 17.78%, and the metagenomic next-generation sequencing (mNGS) positive rate was 57.78% ( P=0.014, kappa=0.273). As per the comparison results, the sensitivity, specificity, positive predictive value, negative predictive value, Youden index and misdiagnosis rate were 100.00%, 51.35%, 30.76%, 100.00%, 51.36%, 48.64% and 0, respectively. Conclusions:Metagenomics examination had high sensitivity, which can improve the etiological diagnosis rate of acute BM in children, especially in case of high clinical suspicion of infection.Therefore, metagenomics examination should be selected as early as possible when the etiology cannot be determined by traditional approaches.

ABSTRACT

Objective@#To analyze effect of fractured file removal from the middle third root canal on root fracture resistance using finite element analysis, which provides a theoretical basis for clinical prognosis evaluation.@*Methods@#Two finite-element models were established, the fractured file removal model (fractured file located in the middle third of root canals, followed by ultrasonic file removal and root canal preparation) and the control model (root canal preparation only), and compressive displacement dependencies on compressive force was computed and compared with experimental data for validation. The validated finite-element models were used to analyze the stress distribution differences during the initiation, propagation and completion of the crack between fractured file removal specimen and control one.@*Results@#The critical breaking force of the fractured file removal specimen was 406 N, and the finite element simulation result was 396 N. The critical breaking force of the control specimen was 502 N, and the finite element simulation result was 483 N. The position of crack initiation in the finite element simulation was basically consistent with that in the experiment. The experimental data of compressive test and the results of finite-element computation were in agreement, thus validating the finite-element model. In the process of continuous pressure, the stress distribution of the control root is relatively uniform, and the location of crack initiation and the direction of propagation have a certain unpredictability. Compared with the control root, the stress concentration on the root with fracture file removal was obvious, especially on edges, and the number of cracks are much more. Because of the thinner radicular wall, the crack propagation rate is faster too. Therefore, the overall root fracture resistant is decreased obviously.@*Conclusions@#During the fractured file removal procedure, amount of dentine removed should be minimized, and the edges and corners which caused by fractured file removal should be shaped to smooth in order to reduce the stress concentration and prevent the root from fracture.

ABSTRACT

Objective Cerebral microbleeds (CMBs) are important indicators of cerebral small vessel disease .However, it is still unclear whether endothelial dysfunction is involved in CMBs .The aim of this study is to investigate the correlation between CMBs and soluble E-selectin (sE-selectin) in patients with acute ischemic stroke . Methods Based on the results of MRI (3.0 T) susceptibility weighted imaging , we divided patients with first acute ischemic stroke into a CMBs group ( n=63 ) and a non-CMBs group (n=63), and recruited another 45 volunteers with normal MRI findings as controls .We collected and conducted comparative a-nalysis on the demographic data , biochemical variables ( including the sE-selectin level ) , vascular risk factors , and the number of CMBs of the patients . Results Ordinal logistic regression analysis showed a significantly positive correlation between sE -selectin and the number of CMBs (OR=1.062, 95%CI:1.023-1.103, P=0.002), higher systolic blood pressure associated with more CMBs (OR=1.014, 95%CI:1.002-1.025, P=0.021). Conclusion Serum sE-selectin is significantly positively correlated with and can be used as a biological marker for the severity of CMBs .

ABSTRACT

OBJECTIVE@#To investigate the significance of human thioredoxin-2 (TRX-2) in monitoring minimal residual disease (MRD) in acute leukemia (AL).@*METHODS@#We used real-time quantitative PCR to serially quantitize TRX-2 expression levels in the bone marrow of AL patients at diagnosis (n=68), at complete hematologic remission (CHR, n=57) and at relapse (n=25). Another 25 normal donors served as normal controls. The upper limit of the bone marrow at 91 was regarded as the reference. TRX-2 expression level at CHR with 91). TRX-2 level was correlated to the expression level of MRD.@*CONCLUSION@#TRX-2 may be the marker for AL and used in MRD monitoring.

ABSTRACT

Cytokines involve in many diseases. It' s reported that reduction or increase of cytokines is an important factor to lead to immunological diseases in nervous system. Other studies have found that certain cytokines in the different stages of the same disease play a different role. Understanding the specific mechanisms of cytokines in autoimmune diseases of the nervous system is necessary for providing theoretical basis of new ways to treat these diseases. This article will review the progress of some main cytokines in nervous systemic immunological diseases.

ABSTRACT

The expression of NF-kappaB is considered to be involved in the progress of neurodegeneration. It has been reported that Nanog can suppress the expression of NF-kappaB. To inspect and verify this finding, we constructed lentivirus (LV) vector that overexpressed the Nanog gene, infected mouse mesenchymal stem cells (mMSCs), and examined the influence of Nanog overexpression on NF-kappaB gene expression. The plasmid pNL-Nanog-IRES2-EGFP was constructed by double digestion and genetic recombination. Sequencing results confirmed that our cloned Nanog gene in the PNL-Nanog-IRES2-EGFP plasmid was consistent with the sequence reported in the GenBank. The three plasmids: pNL-Nanog-IRES2-EGFP, HELPER, and VSVG were cotransfected into 293T cells to produce LV particles. After co-transfection of the three lentiviral plasmids, green fluorescence was observed confirming successful transfection. The mMSCs were infected by the LV and the expression of Nanog was then also verified by the presence of green fluorescence. Nanog expression levels in the mMSCs were examined using Western blotting. Expression of NF-kappaB was also examined using RT-PCR and Western blotting, and in addition with fluorescent microscope after immunocytochemical staining. The levels of Nanog protein expression in Nanog-mMSCs were significantly increased, and the levels of NF-kappaB mRNA and protein expression in Nanog-infected mMSCs were significantly lower than those of Mock-mMSCs and the mMSCs control groups. Our findings suggest that mMSCs genetically modified to overexpress Nanog can lead to the suppression of NF-kappaB expression. This suppression of NF-kappaB could have important implications for the treatment of neurodegeneration, and hence further scientific investigations of these interactions will have significant impact on future clinical attempts to attenuate disease progression.

ABSTRACT

Objective To establish the animal model of damp-heat syndrome(DHS) and explore the expression of aquaporin(AQP) 3,4 gene in gastric mucosa.Methods Thirty SD rats were randomized into 3 groups and observed for 20 days.Group A(normal group,NG) was fed with routine method.Group B(Pi deficiency syndrome group,PDS) was fed alternatively with routine method and aqua of cassia angustifolia Vahl.Group C(DHS) was fed with fat,honey liquid and treated with damp-heat environment.The expression of AQP 3,4 gene in the 3 groups were determined by fluorescence quantitative polymerase chain reaction(FQ-PCR).Results The symptoms and signs in Group C were simiar to the clinical DHS.Expression of AQP3 gene in DHS was statistically significant higher than that in PDS and NG(P

Objective To observe the effect and study intervenient action of Kuijieling Granules (KG) in treating ulcerative colitis (UC) during the active period by the clinical experiment. Methods The patients of active UC (damp-heat syndrome) were randomly divided into 2 groups (KG+SASP group and SASP group) to observe pathological changes of the mucosa,syndrome effect and main symptoms scores,and the protein expression of TLR4,CD14 and NF-?B p65 were detected by immunohistochemistry. Results KG+SASP group had better effect than SASP group in syndrome and mucosa (P0.05). There were outstanding differences among the pathologic grade of UC about IA of TLR4,CD14 and NF-?B p65. ABSTRACT

【Objective】 To establish preliminary diagnostic criteria of spleen-stomach damp-heat syndrome (SDS) in chronic superficial gastritis (CSG) by disease-syndrome combined differentiation method and modern epidemiological method. 【Methods】 A clinical questionnaire of SDS in CSG was drafted out based on literatures study and experts' survey. Questionnaire investigation was performed in 146 cases of CSG, which consisted of 81 SDS cases and 65 nonSDS cases. Mter the investigation, discriminant analysis was adopted to set up the discriminant functional equations and to perform frequency analysis. According to the results, the diagnostic criteria and the simplified diagnostic criteria of SDS in CSG were established. 【Results】 The preliminary diagnostic criteria of SDS in CSG were as follows: (1) the major symptoms were characterized as yellow and greasy tongue fur, fullness or distension or pain in the stomach, loose stool and poor appetite; (2) the secondary symptoms were characterized as bitter taste and sticky mouth, chest depression, thirst but unwilling to drink, fatigue and nausea; (3) diagnosis could be confirmed by the existence of yellow and greasy tongue fur accompanied by either 2 of the other major symptoms, or by either 1 of the other major and 2 of the secondary symptoms, or by more than 3 of the secondary symptoms. The simplified diagnostic criteria were characterized the co-existence of yellow and greasy tongue fur and fullness or distension or pain in the stomach. 【Conclusion】 The preliminary diagnostic criteria of SDS in CSG are scientific, objective and practicable to some extent.

ABSTRACT

[Objective] To investigate the pathological features, secretion of interleukin-8 (IL - 8), tumor necrosis factor alpha (TNF-?) and secretory immunoglobulin A (sIgA) and helicobacter pylori (HP) infection in gastric mucosa (GM) of chronic superficial gastritis with spleen-stomach and damp-heat syndrome (SSDHS) and to explore their correlations. [Methods] Fifty-five cases of chronic superficial gastritis included 35 cases of SSDHS (Group A) and 20 cases of spleen-deficiency syndrome (Group B). Fifteen asymptomatic volunteers served as normal control group (Group C). Pathological changes, HP infection rate, levels of IL - 8, TNF -? and sIgA in GM were observed. [Results] HP infection rate was higher and inflammatory change was more obvious in Group A than those in Group B and Group C (P 0.05). [Conclusion] Gastric mucosa of chronic superficial gastritis with SSDHS presents inflammatory changes, which is related with the increase of IL - 8 and TNF -? secretion and the decrease of sIgA, and HP infection may be one of the factors inducing and enhancing the pathological changes.

ABSTRACT

[Objective] To study the relationship between syndrome patterns of traditional Chinese medicine and bcl- 2 oncogene, p53 suppression gene mRNA expressions in precancerous lesions of gastric cancer (PIGC). [Methods] Forty PLGC cases were endoscopically and pathologically confinned, including 24 cases of moderate dysplasia of gastric mucosa, 9 cases of severe dysplasia, 7 cases of incomplete colonic intestinal metaplasia. Of the 40 cases, 10 were complicated with Qi stagnation, 12 with stomach-heat, and 18 with blood stasis, mRNA expression of bcl- 2 oncogene and p53 suppression gene were detected in situ by molecular hybridization method. [Results] The mRNA overexpression of bcl- 2 oncogene and p53 suppression gene were found in PLGC, and the expression was gradually increased with the progress of lesions. In the complicated cases, the mRNA expression of bcl- 2 oncogene and p53 suppression gene were the least in the cases with Qi stagnation and less in the cases with stomach- heat than in the cases with blood stasis. [Conclusion] Abnormal mRNA expression of bcl- 2 oncogene and p53 suppression gene were found in PLGC cases and related with different complicated cases, indicating the specificity of different syndrome patterns.
